Simple steps to get best patient care experience

1. Go to Book an Appointment page
2. Get an appointment.
   1. Two 15 minutes slots for new patients visit for $300
   2. One 15-minute slot for follow up visit for $150.
   3. Choose from in-person or telemedicine visit.
   4. Home Visits only by prior arrangement and authorization for only $500 per visit.
3. Fill in your information as much as possible to help us understand the reason for the visit.
4. Sign all consents and disclosures forms.
5. Photograph will be needed for identification.
6. Pay fees in full before visit. We only accept credit card, e-check, and or Zelle payments. Any other method of payment with prior authorization only.
7. After in-person or telemedicine visit, patients can see, download, or print visit summary and superbill from Patient Portal.
8. Antibiotics Prescriptions electronically sent to the pharmacy of choice.
9. Labs requisitions ordered electronically go to LabCorp and or Quest Diagnostic labs.
10. Radiology requisitions can be downloaded and printed from Patient Portal.
11. Patient handout regarding specific infection can be found at www.cdc.gov
12. Consultation referral information will be provided via Patient Portal.
